Liandry's Torment was released with the Season 3 items as a weaker version of the Blackfire Torch item from the Twisted Treeline and Dominion. It, however, has the same basic function; when you hit an enemy with a damaging spell, it applies a damage over time effect based on the enemy's health. The most notable difference is that Liandry's Torment uses Current health instead of Max health. However, they both have the same basic utility; they make any ability a very strong source of damage over time. Liandry's has the caveat that it is only effective against targets with full or nearly-full health. This means that its greatest utility is poking high-health targets to 2/3 or 1/2 health.
So then the next question is, how can someone apply Liandry's Torment to an enemy at full health (generally, an enemy that is staying out of easy engagement range) in order to use the item to poke enemies down before an engagement? This article describes obscure and bizarre methods of landing Liandry's Torment on enemies from a safe range. Note that for most of these, a Rylai's Crystal Scepter will apply a slow, doubling the effectiveness of the Damage over Time effect.
Long Range
These abilities have exceptional range and therefore can be used to safely apply the Liandry's DoT. I've only listed champions who potentially benefit from the magic pen and the ability power, so you won't see a suggestion to build AP Urgot here.

Delayed/Persistent Effects
These effects take significant time to land or stay in effect for a while, making them effective at denying an area or landing the DoT on chasing/approaching champions.
